Wound Care Referral

Wound care clinic at St Vincent’s is scheduled on an “As Needed” basis and based on volunteer availability. Please use the above form to add patients to the waitlist. The patient will be contacted as soon as a date is set for the clinic. Waitlist is about a month long for scheduling.
