Here you can find our antibiotic core formulary. This list can assist our team in optimizing patient care and ensuring the responsible use of antibiotics. It shows what we currently have available in our pharmacy closet and may be able to provide to patients who are unable to afford them.

  3. Azithromycin 250 Mg Tab
  4. Cefadroxil 500 Mg Cap
  5. Cefdinir 300 Mg Cap
  6. Ceftriaxone 500 Mg Im
  7. Doxycycline Hyclate 100mg Cap
  8. Levofloxacin 500 Mg Tab
  9. Metronidazole 500 Mg Tab
  10. Nitrofurantoin (Macrobid) 100 Mg Cap
  11. Sulfamethoxazole / Trimethoprim 800-160mg Tab

Common Indications:

 AMOXICILLIN TAB 500 MG TAB

  •  Selected GAS infections – Strep Throat
  • Community Acquired Pneumonia (CAP) in combination with atypical coverage (high dose required)

 AMOXICILLIN/CLAVULAN 875-125 MG TAB

  • Otitis Media, Sinusitis
  • Odontogenic Infections
  • Bite Wounds
  • Sea water skin and soft tissue infections in combination with doxycycline (Vibrio)
  • GI Infections – diverticulitis
  • Community Acquired Pneumonia (CAP) in combination with atypical coverage

 AZITHROMYCIN 250 MG TAB

  • Atypical Pneumonia in combination with typical coverage
  • Chlamydia Trachomatis – one dose, if don’t feel like 7 day course of doxycycline is practical
  • QTc risk, check if risk factors
  • NOT first line for GAS pharyngitis or CAP due to unnecessarily broad- spectrum and increasing resistance among streptococcal species 

 CEFADROXIL 500 MG CAP

  • Skin and soft tissue infections, where MRSA not suspectedThis is in place of Cephalexin (Keflex) due to preferred once daily dosing

CEFDINIR 300 MG CAP

  • Complicated UTI
  • Community Acquired Pneumonia (CAP) in combination with atypical coverage
  • Complicated Otitis Media
  • NOT first line for GAS pharyngitis or CAP due to unnecessarily broad-spectrum and increasing resistance among streptococcal species

CEFTRIAXONE 500 MG IM

  • Gonorrhea
  • Complicated UTI
  • Complicated Otitis Media

DOXYCYCLINE HYCL 100MG CAP

  • Abscess and Skin Infections concerning for MRSA
  • Sea water skin and soft tissue infections in combination with Augmentin
  • Chlamydia Trachomatis
  • Atypical Pneumonia in combination with typical coverage
  • Advise of phototoxicity
  • Risk of Esophagitis, take with a full glass of water and sit-up 30 minutes after taking
  • Does Not have good Strep coverage, don’t use for monotherapy of skin infections if concern for Strep

LEVOFLOXACIN 500 MG TAB

  • Pyelonephritis
  • Community Acquired Pneumonia – high risk
  • Concern for Pseudomonas
  • Use sparingly due to concerns for promoting resistance and side effects including QTc prolongation, concerns for cardiac safety, tendinopathy (black box warning), neuropsychiatric effects, high C Diff risk

METRONIDAZOLE 500 MG TAB

  • Anerobic Infections
  • Trichomonas
  • Bacterial Vaginosis
  • C Diff Colitis (mild)

NITROFURANTOIN (Macrobid) 100 MG CAP

  • Uncomplicated UTI / Cystitis
  • Do NOT use if concern for pyelonephritis or if GFR < 30

SULFAMETHOX/TRIMETHO 800-160MG TAB

  • Abscess and Skin Infections concerning for MRSA, has limited strep coverage though not preferred
  • UTIs
